and precautions for each test on the “Test Items” page.

Please fill out the downloaded request form.

After filling out the hospital name, veterinarian’s contact information, email, etc.

on the “Submit Test Request" page, upload the completed request form as an attached file.

Please feel free to fill out the patient’s symptoms,
other information on the sample, or any inquiries, and click the [Submit] button.

When the submission is completed, the details are automatically sent via email.

After checking the sample delivery method, please send samples.

Test results will be sent via the email you entered at the time of submission.
